Statement of Intent

As part of our contract with NHS England we are required to put in place several new initiatives in relation to your medical records.  We have advertised on our website and in the surgery, information regarding Summary Care Records.

Summary Care Records

From 31st March 2015 every practice will be required to provide a regular upload of patient information to the Summary Care Record (SCR).  The SCR will only be viewed in emergency or urgent settings with patient consent unless you are unconscious.

You are able to opt out of this service by attending the practice and completing the form, alternatively the form can be accessed here

Patient Access to their medical records via SystmOnline

GP practices are now required to promote and offer the facility for patients to view information from their medical records i.e medications, allergies, adverse reactions and any information that has been agreed between the GP and the patient.  If you do not already have access to request your medication and make appointments online please attend the practice and request a username and password for SystmOnline.  You must have photo proof of ID for the reception team to be able to set this service up for you.

GP2GP record transfers

All practices will have a facility of transferring your medical records between your old practice to your new practice.  This is already available if both practices have the same GP clinical system.  A new service in now available for the electronic transfer of medical records to take place between two different clinical systems.  This is called GP2GP.  All GP practices should be live with this by 31st March 2014.  This will speed up the transfer of patient information when a patient registers with a new practice.
