Cancelling your Appointment

 

If you are unable to attend an appointment with one of the doctors or nurses, please telephone or use the link at the bottom of this page to cancel your appointment.

By giving us as much notice as you can you are helping us to make sure that someone else is given your slot.

Change of Address

Please let us know if you change of your name, address or telephone number or any other circumstance which may affect the practice.

If you move to an address outside the practice area, we will ask you to register with a practice that covers your new address.

Travel Vaccinations

If you require any vaccinations relating to foreign travel, it is important to make these arrangements at least 6 weeks before you travel.

Please complete the form below and return it to Reception as soon as possible.  Our receptionists will then book you an appointment with a nurse.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS.
