Cancelling your Appointment

 

If you are unable to attend an appointment with one of the doctors or nurses, please telephone or use the link at the bottom of this page to cancel your appointment.

By giving us as much notice as you can you are helping us to make sure that someone else is given your slot.

Change of Address

Please let us know if you change of your name, address or telephone number or any other circumstance which may affect the practice.

If you move to an address outside the practice area, we will ask you to register with a practice that covers your new address.

Registration with the practice

When you register you will be asked to fill out a Family Doctor Services Registration Form. You will be asked to sign this form to confirm that the details are correct.
